Maine certification guide
Getting certified in Maine is a short list. Here it is.
To start, choose one of two ways to be certified, then text us at (207) 241-2815. Relief Clinic LLC has certified Maine patients from Auburn since 2015, and Same Day Approval is the aim. Telemedicine copies are electronic and uploaded the same day.
Confused with which process you should go with? We can help. We offer FREE consultations, and you are welcome to ask before you decide.
Step one
How would you like to get certified?
Telemedicine
$43.99
Get certified by a Relief Clinic provider through your mobile device or computer. Telemedicine is available seven days a week, 10:00am - 8:00pm, and it runs best on an iOS or Android phone. Visits are not recorded. Electronic copies are uploaded the same day.
How the video visit worksIn person
$39.99
You'll see and consult a Relief Clinic provider directly, at 216 Center Street in Auburn, on Wednesday from 12:00pm - 3:30pm or Friday from 10:00am - 2:30pm.
Visiting Center StreetVisit fees for both Maine options are on the pricing page. Already certified and due to renew? Telemedicine is available for recertification too.
Step two
What to bring
State of Maine: bring your Maine State ID, or driver's license.
Maine law requires that a medical provider physically see the patient's Maine Drivers License or State I.D Card before approval. If there is no proof, certification will not be granted, so keep your ID within reach for a telemedicine call as well as for a visit to the office.
How to prepare
Medical records. They are NOT required in the State of Maine.
That is a Maine rule. Florida works differently, and the clinic's Florida offices do ask for records, so keep the two states separate. Patients with anxiety, depression, PTSD or any other mental health condition have one more thing to do, which is covered at the end of this page.

Step three

One more thing
Mental-health patients fill out a form
All patients with anxiety, depression, PTSD or any other mental health problems have to fill out the form. That means the HIPAA form and the PCL-5 questionnaire.
Relief Clinic LLC will not share your information with anyone, and we follow HIPAA laws. Both forms are on the forms page.
